1. A photo of yourself holding a photo ID in the frame of the camera lens. You may redact any information you don’t wish to share except your name and date of birth. The photo ID image must be legible.
2. A reference from a previous provider with an online presence I can contact. I’ll need their name, phone number or email, and website or advertisement. Please be courteous and let them know I’ll be reaching out.
